The iWatches with the ability to tie directly into cell services don't need a cell phone to make the call. Cost due to the cell service is more though.

Apple Watch for fall detection without an iPhone because an iPhone is required for the initial setup, management, and software updates, though a GPS + Cellular model can call emergency services independently after setup if you have service, without the iPhone nearby for calls. The watch relies on the iPhone's connection (cellular/Wi-Fi) for emergency calls, unless it's the cellular model, which uses its own connection, but still needs the iPhone to get started.
 
I "Think" - if you are near an Apple Store or even an ATT, or Verizon or T-Mobile, Get a Manager and I "think: they may be able to set it up. It does need to be an Apple Watch WITH cellular ability.

If the words "Apple Watch" is used it might respond in the negative but if "Apple CELLULAR Watch" is used , then yes it can after it is set up. Go to an Apple Store or ATT, Verizon, or T-Moble and ask for someone who has done this.
 
We were recently on a cruise on the Nile in Egypt. A fellow traveler (in his late 80's) got up and started dancing with the Belly Dancer. The music was loud, he's jumping around having a great time, and suddenly his phone goes off. When he answered it was his family back in California very concerned and asking if he was OK. His watch had gotten confused and thought he had fallen, and sent a distress call back to his family.😂
 
What about those push a button things and you wear around your neck. You can talk over them too. Forgot the name but you see the commercials all the time.

Those work too but you have to be able push the button and or talk, where as "fall detection" dials 911 and family with the precise location even if you can't or are unconscious.
