It looks like a piece of rod or a bolt through the cranium. Did they survive? Keep the eye?

I am going to guess that it is a Paper mate pen with the cap still on. Whatever it is, it does not look like a good situation.

A metalic object attached to a wooden handle. Had to be traveling at a high rate of speed. Wound could be survivable since did not cross the mid brain, although quality of life will suffer greatly.

Because of the light at the end, I would GUESS that is metal and the shaft is not.
So, I would be led to surmise an arrow????

This patient presented for a failed suicide attempt with a pen. The axial CT scan shows an entire ball-point pen.
As Ed pointed out, the metal tip of the pen refracted the beam. The pen missed the optic nerve, middle cerebral artery and any eloquent brain. The pen was removed under fluoroscopic guidance. Upon removal, there was bleeding from the ophthalmic artery. Endovascular embolisation of the bleeding vessel was performed with coils with a good result. It ended well as the patient retained both his vision and life.

I believe this assumes there is some sort of brain functioning BEFORE the injury. In this case that COULD be an erroneous assumption
Pretty crazy if you ask me. This makes the first time i've heard about a suicide mission with a bic. I'm sure there were some knives in the kitchen.

I havent been able to carry one of my pens on a plane since 911! Last time I traveled I had two portfolios in the hold!
I immediately thought of Phineas Gage when I saw the x-ray! Poor guy gave himself a frontal lobotomy with a steel tamping rod while working on the continental rail road........it completely altered the man and his behavioral issues, caused by the accident, destroyed his life! I hope this PT has a better outcome.
Guys you dont know just how crazy folks are till you have worked an ER......and they are the loonies that are free!!!!
