This is a new version of a pen turning mandel.

Don't ask how it happened:(. Lucky I have more mandels. Broke my finger and beat up one of my more expensive blanks. Can repair that.

Turned off the lathe. I was too hot and tired. Should not have been in the shop.

Ouch! hope our finger will be better soon, Glad it only got the finger. I've had this happen when the tail stock sliped before. I didn't get as much slice in my mandrel though.

I am curious how this happened too! Whenever I get a catch, the blank usually just stop spinning as it slips from the spinning mandrel. It only happens as I am rouging/rounding the blank and get too aggressive with the gouge cuts.
 
Hey, since you guys started asking I would like to know too. Sometimes things become so routine that maybe we don't pay attention to everything we should. This could be an eye opener for all of us if you are willing to share.
