It was the last twist of the clamp.

Here's what happened because I forgot to open up the finial tube so there wouldn't be much pressure needed for the finial and clip:

I don't have enough left over chinaberry so I'm going to have to do a black resin grip. I thought about contrasting woods, but nothing else would look right with that beautiful wood.

Man I hate when that happens. I was turning my first one piece slimline today and had it break right in the middle (worthless wood cast). Ruined my whole day too.
 
I messed up one to. One very simular to yours. I must have drilled the hole ever so slightly off but still went ahead and when i went to assemble, it was to tight. Forced it on and now i can`t get it to twist. Oh well.
 
I was turning a cross grain pen yesterday and got in a hurry. It was getting late and I was tired....I got a little too agressive with the tool, or didn't sharpen it enough, or.... Of course it shattered into a dozen pieces. Oh well.
