Well here are some pictures of my blowout. Those pieces were gone and so I had to improvise and it turned out real good...

If you ask what I used to fill those big holes with, I might tell you or better yet. Why don't you take a guess!

My experience with blowouts tells me most is caused by the wood absorbing the glue too much when installing the tubes. Your wood looked like a good candidate for that. I figured out if I wick thin CA into the sides of the drilled hole, rotating to completely cover the insides, then redrilling after it cures I get a much more solid bond to the tube. You do have to be quick putting the tubes in, for some reason the CA sets up much faster after this treatment.
