Does anyone know of a drawer slide that is full extension the mounts on the bottom of the drawer? I would even use 2 centermounts if they were full extension. I just don't like the tandem type because they take too much height from the drawer. I found some industrial ones but the price is way out there.
I also considered using the side mount but I've had those come apart from twisting and I'd be afraid they may fly apart if too much weight is put on them.

I don't have experience with that vendor. They were listed as the source for Hettich brand of undermounts in a project featured in the March 2016 issue of WOOD magazine.

You're likely limited to a few brands regardless of the vendor.
 
What about this one? This can be flat mounted and is little thinner.

$43 per set...ouch!

I can find lots of places to buy slides, just not the ones that would be a good fit for my project.
I did some additional comparison and it seems the center 3/4 extension slides are made the same way the others are but they downgrade the load rating by a factor of 4.
I'm going to prototype with a set of KV 8400 and see how it works.

Take a look at Custom Service Hardware here: Wholesale Hardware, RTA Cabinets, Rolling Ladders & More | Custom Service Hardware . They have a wide range of full extension, soft close, etc. They do Blum, Hettich, GSlide, Knape & Vogt and their own brand. I used their house brand when I did our kitchen expansion - full ext undermount with soft close. Amazing prices when compared with "name brands". Pretty much the same issue concerning drawer height but I built the cabinets and took that into consideration in design.

Side mounts come in a variety of design weights from all the Mfr's. But more design weight, more $.

I like CSH because of their selection and their prices but the cust service is awful good, too. I've had occasion to call them a couple times. Nice to speak with folk that really know their products and are looking to help.
