Defective M13x.8 tpl lead die

I purchased an M13x.8 tpl lead die in the recent group purchase. The die only cut 2 threads and leave a space where the third thread should be. I borrowed the same die from a friend who also purchase one during that group purchase. His die cut perfectly. I then tried my die again with the same poor results.
I've contacted Tapco but they can't help me without the original paper work of the group. Hanna has that information but I have not been able to contact her. Does anyone know how to contact her or someone else at IAP that can help me.
