Can you identify this pen kit?

I had a customer ask if I could repair this pen even though I did not make it for him. I don't recognize the pen kit and I have not been able to find this exact kit at any of the suppliers I use. It looks familiar but I am stumped! Anyone know what this kit is and where I can get one to repair it (plastic threads broke off inside one of the barrels)?

You are welcome. I hope you can save it - it must mean something a little extra to the customer if they are looking to repair it.
