Does anyone have an order form I can use for pen orders?

Sorry if this is the wrong place to post guys, but I need a good order form for some customers.

You can make one in Excel or Word. All you need is info that you understand. Make/style of pen type of material and any customization (engraving, different clip style, etc.)

You can purchase a SALES FORM receipt booklet from an office supply store such as Staples or Business Depot. Essentially they are a blank carbonless forms that you insert information such as price/style/quantity/applicable taxes/date etc. Very inexpensive way of having a sales record both for you and your customer.
