Need image placed on blank...

I've got an order for a sierra type pen. I need a blank with a personal image placed on the blank. I've never done this type of blank before. It is needed before Christmas. My problem is that I've come down with the flu & I really want this order to be ready to go out before Christmas. I'm not sure what is the best way to create this...print image on photo paper & wrap around blank like a stamp pen or can it be done on a water slide decal.
