A single start makes one thread - the top and bottom will match up every time.

A triple start makes three sets of threads at one time - the top and bottom will line up 1/3 of the time.

It usually takes fewer twists to screw a cap on with triple start threads.
 
note if you're trying to line up a pattern on the pen body and lid, then you would use a single thread to ensure the lid lines up every time
 
Triple start is what most people expect in a pen. It secures faster, the seal is stronger. On the down side it only lines up with the pattern 1 out of 3 times, like Brad said.
