Was the wood dry??? I have only had this problem happen once and just recently. The wood was an Austrailian wood I had gotten off Ebay. Viscolate or something to that effect. Pen looked great when finished but after a few days things when wacky. The cracking was the wood itself. Suspect it was wet and I didn't let it dry prior to turning. I use thick CA and usually 3 thick coats, sometimes 4, and all are set with pump accelerator with no problems.

I would tend to agree with Greg, It's only happened to me with wood that was not dry.
