I had seen that the other day. I've never worked with a concrete saw so I am curious how that blade fits on the tool.

Did the blade come off the shaft (lose the Jesus nut) or did the shaft break? Can't see through the hole.

I believe that a worker forgot to check the tool before using it & the blade came off due to a loose nut.

IIRC it said it was partially due to operator error.
