Use a slightly smaller punch that fits through the coupler at a slight angle, so it can catch the lip of the insert on the opposite side. Of course, once one side is out, use a larger punch that fits the tube to get the other end out.
 
Be very, very careful punching at an angle, a brittle blank will crack on you if you flex or "egg shape" the tube at all.
Small taps as you move the punch around will let you "walk" the piece out.
 
If you have a JR series's rollerball nib assembly, screw it on and use the largest punch that fits through the the threads to knock out the end with the RB nib in it. The use the punch closest to the tube size to knock the other one out.
