Curtis is ready to prepare the entry forms for those of you who will be using them. The basic form is ready, all he has to do is customize them for the individual contests.

PLEASE POST THIS INFORMATION IN THIS THREAD SO CURTIS WILL HAVE ONE PLACE TO LOOK FOR IT. Don't send it by PM, post it here.

Curtis needs to know:

Contest name
Chair person's name,
What entry information do you beed on the form (i.e. Contestants information, mailing address, email address, etc.)?
Do you need a place for any type of narrative?
If your contest requires the members to submit pictures how many slots for pictures do you need?
What email address should Curtis use to forward the entry forms to you?

In summery, picture your entry form and post the description in this thread for Curtis to find.

I will need an entry form for the
"Kitless Pen Contest"
It needs a place for 3 pictures and a place to describe the entry in addition to the personal info.
Pretty much like your example.

I will also need an entry form for the
Youth Pen Contest
Childs Name.
Members Name its submittied unde
3 pictures and a box to describe the pen.
It will need to state something to the effect of
" Describe the pen and the work done by the entrant and any work done by an adult."
as well as the Adult member that it is submitted under.

Puzzle Contest

Curtis,
I sent you some 2014 Featured Pen photos for use as jigsaw puzzles. Let me know if they are ok or if I need to select some different ones.

I'm thinking that we should ask the entrants to identify the pen & its maker by entering the Pen Title, User ID & Member Name after completing the puzzle.

Entrants should supply their User ID, Member Name and shipping address.

I'm open to suggestions since I haven't actually seen a puzzle contest before.

1) Allow three photos to be uploaded.

When I talked with Curtis yesterday he told me that he was too buried to do the puzzles. This post indicates that he will be creating them. Maybe I miss understood but can we clarify who is doing what in regard to puzzles?

Mike,

I will create ones that Edgar sends me and get them online. What I had a hard time with last year was coming up with the puzzles to begin with. Creating good word lists for crossword, for example, is difficult and time consuming.

Photo Contests

Curtis, Not sure how difficult it is on your end, but in the past there was one entry form for all the photo contests and a drop-down or check-box to indicate which one you were entering. I would prefer separate forms for each contest, as I had several people confused by it. If it is a huge issue, then we will all get by just fine with how it was set up last year...

Do you need a place for any type of narrative?

No. In fact it would be great if there was not one. Some people submitted rather extensive explanations of their photos that I felt bad I could not post with them...

1 (ONE)
(ONLY ONE!!!)
(Did I mention they can only submit ONE PHOTO???)
(I ain't makin' that mistake again!!!)

What email address should Curtis use to forward the entry forms to you?

Then duplicate the same thing for the Shop Life Photo Contest, or set it up with an option box if that is significantly easier

I have spoken with all of my winners. One was Classic Nib (Roy and the winner were in direct contact and prize has been shipped), 2 were Doug's Woodworking (both of them have been given the information on contacting Doug per his request), there was one Alumilite (I believe I got the info to you already), and the rest were BOB's (the shipping has already been coordinated).
If you think you may be missing my Alumilite winner, please let me know, and I will send it over to you immediately.
