Anyone that uses a vacuum chuck for reverse turning bowls, please check your rotary adapter regularly for dust and debris which could cause it to seize. Also, don't attempt to start a lathe without a good quality faceshield. Before you start your lathe, ensure the piece you are working on is held securely, held securely, held securely. I just can't say that enough. You might step away from the lathe to do something and forget to secure the piece or forget to check if the piece will foul the rest.
There have been a number of deaths and serious injuries over the last few years from woodturning.

Some people tend to bring up the tailstock and not tighten it fully. Any vibration can cause it to creep back, especially with a piece that isn't true to begin with.

The fact is, a quick watch of those few seconds shows several things that he did that were contrary to good safety.

I know. As he was walking into the firing line, his gouge almost hit the bowl. That's what I thought took the bowl off the lathe. Plus, the area around the lathe looks very cluttered.
