Adding to my July 20th post. I was preparing to drill a blank for a tool handle when I noticed the chuck body was loose, rocking back and forth on the adapter. Thought the adapter might be loose but it showed to be solid. However, I noticed a tiny hole in the chuck body. Cleaning out the end showed a 3mm set screw (grub screw to the Brits). Tightened it and cured the shake. Not sure if that set screw seated on the adapter or where else. How many of us take time to go over chucks, headstock, quill, banjo, etc. to make sure everything is secure? Lathe secure to the base or whatever it sits on?
 
If there is a flat on the shank/shaft of the adapter, make sure the set screw is biting on the flat and not the turned round surface.
 
Couldn't get the adapter out. I had removed the only set screw that is visible through the outer case. Tried CW and CCW but didn't want to force it.
