This is a Personal Pen in chrome, from Wooden Quill. The blank, a label cast, is for nurses. It is also from Wooden Quill. This is the one I had chatter problems with. In spite of the problems, it turned out nice. It's a Christmas present for my daughter-in-law, who is a nurse in a maximum security prison. I don't know how she does it. It's hard to see in the photos, but it says, "Doctors Diagnose. Nurses Cure."
