There was a thread the first part of April in which the question was raised about modifying a kit to use the refill for a Fisher Space Pen. I said it seems to be the right length for a Sierra, using the Fisher-supplied adapter, and another member pointed out that the adapter will slide further down the refill, causing the pen nib to slide up into the grip. I verified this by making and then using the pen for a day, in which that problem became obvious.

The white pen tube is from Woodcraft, and it's sized for the standard, non-click Wallstreet II. The small adapter shown above the Fisher mechanism is a nylon 3/16" sleeve. When cut to slightly less than 1/4" it slides inside the Fisher part, preventing upward movement of the refill. Once the 1/4" spacer is installed, it does not come out easily. I've used it for over two weeks without incident.

I apologize for the poor quality of the pictures.
