advice securing clips

I have a question regarding the upper cap that holds the clip on designer pens. How do you keep them from coming loose? CA glue, loctite, just tightening by hand etc.

I just tighten by hand. However, before assembly, you could tighten the insert with a pair of pliars. I haven't had much trouble by hands.
