Have you tried Kallenshaanwoods.com, I don't know if they have it but I think they can make anything.

ExoticBlanks.com has a large number of boxes similar to that with a variety of engravings. Look in their Kallenshaan section.

Lazerlinez.com may also, and Woodnwhimsies.com is worth a look.

Kallenshaan will do custom boxes of that style with or without windows.
