First use of the tap guide in my metal lathe resulted in it getting stuck in the tail stock and would not self eject due to it being too short. Made the following modification by removing the allen plug and replacing it with an allen head cap screw. After I took the pics I noticed that it was now a little too long so I shortened the bolt.
