Navigator rollerballs end up crooked

Has anyone experienced a problem with rollerball pens (particularly the Navigator, in my experience) ending up crooked when the cap is on? Is this caused by an error in assembly? I thought that perhaps the connector that contains the thread was pushed in unevenly. But in looking at that connector, it does not appear uneven. I've made several of these pens recently, and half of them come out this way. Any thoughts?
