Transmission not locking

Every rare once in a while, I've had a twist pen transmission that doesn't lock
and the refill will retract while the pen is in use.
The upper barrel isn't slipping on the tranny; it just doesn't lock.
Short of disassembling the pen and replacing it, are there any other fixes
to make a twist pen transmission lock?

I've had it happen on Euros when the tranny wasn't pushed in enough to get it into locking position or (say, on a double-twist) the top barrel wasn't exactly the right length, but with your experience, I'm sure you've long since thought of that.

When I first started I had that happen on slimlines from putting then in too far. I also feel you are probably far enough advance now to recognize this. But just in case or for somebody new how I figured it out was to pull the final barrel off, pull the filler out , twist the trans by hand and then put the filler back in. I then retracted the trans and then tried to see if I could screw the filler in farther. In my case I could which ment the trans was in too far and the ink filler was bottoming out in the nib before the trans got to the lock position
