Ordering from China TSCA

Hello all,
First off if this is not in the correct forum, sorry I didn't see one that fit and I tried searching and didn't turn this up.

When ordering a bigger quantity customs stopped my dhl shipment and said I had to fill out this form for some slimline and comfort ballpoint pen kits. I have never filled this out before and wasn't sure which box to check and if I should list anything.

The TSCA form is for the ink in the refills of the kit, and should be filled out by the manufacturer. I do know that when I import refills, the manufacturer checks the first line, and not the second and lists a couple of chemicals.
