Just your regular pretty cobalt blue acrylic from the ebay somewhere.
#6 jowo nib.
And who can guess where I screwed up BIG time? :/

Yep.
I went to bed last night with pen in hand, made a sketch of how to fix it, and this morning I put the cap back on the lathe.

I trimmed the diameter of the cap and finial down a little, and created a little lip for the inside of the clip to ride against. That keeps it centered on the cap!
